Ventricular Fusion Rhythm

Report:

Sinus rhythm

Accelerated idioventricular rhythm (AIVR)

AV dissociation

Ventricular fusion beats

Comment:

The periods of AIVR in the upper strips are introduced or terminated by fusion complexes; the 8-beat run in the bottom strip consists entirely of fusion beats. If it were only a little faster, it would constitute yet another form of semiventricular70 tachycardia!

Idiofocal rhythms are a common association of thrombolytic therapy; they may or may not indicate coronary reperfusion and rarely require treatment. They are not as reliable as ST segment normalisation in signalling successful thrombolysis.

180. 81 year old man with chronic AF, on regular digoxin therapy.
