Comment:
Oddly enough the computer diagnosed âregular rhythm with unusual P axisâ, as though negative P wave in L1 is merely âunusualâ. It also went the full hog: âanterolateral infarctâ and âright axis deviationâ. The trace is diagnostic of mirror-image dextrocardia.
The precordial sequence is very characteristic: diminishing V1.
Important information from true chest leads (in this case, right-sided) may be missed if dextrocardia is unrecognised62. Below (Fig 91a) is this patientâs ECG with right-sided chest leads, revealing Wellensâ warning of proximal LAD stenosis.
