Comment:

The trace suggests mitral stenosis. She had it, but not an ordinary one: it was caused by a large atrial myxoma. In her case, part of Carney’s syndrome54. She had lentiginous spots on her skin, including lips: a good short case for Fellowship exams. Delightfully freckled, I said on a teaching round (in those pre-PC days), but nobody picked up the clue.

Postoperatively, her ECG normalised, except for the borderline – for her age - RAD (Fig 74a).
