Comment:
The voltages are the very large (91mm QRS V4, pure R 12mm V1). Katz-Wachtel phenomenon denotes the large, relatively equiphasic, midprecordial ones.
The child had pulmonary atresia and VSD; There were multiple aorto-pulmonary collaterals and severe pulmonary hypertension, with cyanosis. A shunt with 'unifocalisation' of collaterals were performed three years previously.
