Comment:

Obese young woman with unexplained BP fluctuations and this ECG does not inspire a long differential diagnosis.

The right precordial T wave inversion is not striking, but, as expected, became so the next day (Fig 30a below).

30a. Right precordial T inversion develops some time after the event and implies a large embolus. 31. 39 year old lady on sotalol therapy for atrial flutter, comatose after a VF arrest
